Elizabeth FormanElizabeth M. Forman, 90, of Rushville died at 1:58 p.m. June 26, 2015, at the Memorial Medical Center in Springfield.

She was born Oct. 13, 1924, in Rushville, a daughter of Dannie and Edna Walker Dace.

Surviving are two children, Jane (and Jerry) Curry and Philip (and Norma) Forman, all of Rushville; one sister, Donna (and Harold) Stephens of Rushville; one brother, Charles Dace of Rushville; six grandchildren; eight great-grandchildren; and several nieces and nephews.

She was preceded in death by one son, David Forman; and one sister, Marjorie Bigham.

She worked as a Licensed Practical Nurse for St. Luke’s Health Care in Beardstown.

She was a member of First Christian Church (Disciples of Christ) in Rushville and had attended the former Bethany Church near Camden. She also was a member of the Morning Glory Group and volunteered with Retired Senior Volunteer Program in Rushville.

Graveside services were held Tuesday at the Bethany Cemetery, north of Rushville, with Reverend John Sowers officiating. Burial followed. Wood Funeral Home in Rushville was in charge of arrangements.

Memorials may be made to the Schuyler County Senior Center in Rushville.
