Frank HazzardFrank B. Hazzard, 78, of Lewistown died at 9:03 a.m. Aug. 31, 2009, at Graham Hospital in Canton.

He was born Aug. 21, 1931, in Peoria, a son of Beulah Butler Hazzard and Frank Carroll Hazzard, who preceded him in death Dec. 10, 1930. He married Dorothy Marie Baird on Feb. 2, 1952.

Surviving are three children, Marie Eileen (and Jeff) Mooney of Marion, Ind., Frank Carroll (and Liza) Hazzard of Avon, Ind., and Lawrence Irwin Hazzard of Collinsville; five grandchildren; and four great-grandchildren.

He was preceded in death by his mother, Beulah Hazzard Puckett; one stepfather, Alfred Puckett; and one granddaughter, Jaclynne Suzanne Hazzard.

He was raised in Lewistown and graduated from Lewistown High School in 1949. He attended Western Illinois State College from 1949-51, and graduated from Southern Illinois University, Edwardsville in 1964 with a bachelors degree in government and history.

He enlisted in the USAF at Lackland AFB San Antonio, Texas, in 1951 and transferred to the United States Army from which he retired in February 1973. While in the Army, he did a tour of duty in Korea and two tours in Vietnam. While in Vietnam, he was awarded the Bronze Star with ā€œVā€ for valor for heroism in connection with military operations against a hostile force. In addition to the Bronze Star, he was awarded the Army Commendation Medal and a Purple Heart.

He was a member of Lewistown Presbyterian Church, a member and past commander of Lewistown American Legion Post 1 and Lewistown VFW Post 5001.

Services were held Thursday at Henry-Lange Memorial Home in Lewistown with Rev. Danie deBeer officiating. Burial was in Oak Hill Cemetery in Lewistown with military services conducted by Lewistown VFW, Lewistown American Legion and U.S. Army Honor Guard.
